David J. Polak

DAVID J. POLAK, age 52, cherished son of the late James and Mary (nee Novak); dearest brother of Rachael Olivieri (husband Joe), Daniel (wife Lisa), and John (wife Adriana); beloved uncle of Alessandro, Giovanna, Calvin, Isabella, Juliana, and Madeline; dear nephew, cousin, and beloved friend and confidant to many, passed away peacefully at Hospice House on April 10, surrounded by his family.

David was born June 28, 1973, in Cleveland, Ohio. He graduated from Brush High School of Lyndhurst in 1991 and went on to attend Cleveland State University. After college, David found his calling working within hospice. He enjoyed music, cooking, and photography. David was generous in every sense of the word with everyone he knew. David knew how to dress for every occasion, and his quirky and funny t-shirts always made those around him amused and even impressed.

David is remembered for his caring and helpful spirit, sense of humor, and devotion to his family and friends.

Mass of Christian Burial Monday, April 20, 2026 at St. Noel Catholic Church, 35200 Chardon Rd, Willoughby Hills, OH 44094, at 11 AM. Family will receive friends to pay tribute to and celebrate the life of David at DeJOHN-FLYNN-MYLOTT FUNERAL HOME OF WILLOUGHBY HILLS, 28890 CHARDON ROAD (between Bishop Rd. and Rt. 91) FRIDAY 5 - 8 PM.
